Vitamin D3 in Pain Treatment

A recent personal experience brought me to look closely at what is known about Vitamin D in pain. Below I summarize only a few studies in areas that may be of interest. I will be writing more about Vitamin D in pain. I will cover 25 HydroxyvitaminD and the differences between Vitamin D2 and D3 and how these are metabolized in humans in another article.

Vitamin D is important for bone, cartilage and muscle function. Common manifestations of Vitamin D deficiency are symmetric low back pain, proximal muscle weakness, muscle aches, and throbbing bone pain. Vitamin D2 and D3 are commonly called Vitamin D. Be careful, D2 and D3 are not the same. Vitamin D3 is generally the “right one” to take if OTC vitamins are your choice. I will provide a good deal more detail on the differences and safety in ancoming article.

Laslett et al studied where serum Vitamin D can be used to predict knee and hip pain in adults ranging from 50-80 years. Participants in a study had serum 25-hydroxyvitamin D (25-OHD) levels determined by immunoassay, along with ranked pain at baseline using the Western Ontario scale known as WOMAC. Results shown average total knee WOMAC score was 3.2. Of the participants, 4.2% had moderate vitamin D deficiency at baseline (25-OHD 12.5-25 nM) and most enlightening was that patients with 25-OHD less than 25 nM predicted knee pain. The researchers concluded that moderate vitamin D deficiency independently can predict knee pain as well as hip pain. The correction of moderate vitamin D deficiency may help moderate worsening of knee or hip pain in elderly people provided they have low Vitamin D levels to begin with. If not, supplemental Vitamin D may not be helpful.

In another 2014 study, Mergenhagen et al demonstrated muscle from statins can be traced to low levels of Vitamin D before administration of statin therapy. Here, again, muscle pain was related to and was also shown to be treatable with Vitamin D. Myalgia was seen in patients with Vitamin D around 65nM. The 25-hydroxyvitamin D level in those who reported myalgia was approximately 10 ng / mL lower compared with those who tolerated simvastatin administered at 80 mg.

Vitamin D also appears on at least one other pain front. Empirical data reported by Batchellor suggests a possible relationship between Vitamin D3 deficiency and cluster headaches. The study author makes the economic point that at less than 30 cents a day, treatment of this excruciating headache with Vitamin D may have the potential to become a revolutionary therapy.

Vitamins That Have Value

People who use multivitamins and other nutritional supplements tend to lead healthier lives overall. Vitamins and mineral also help many to be more positive about life. There are those who say that vitamins are a waste of money because people can get all they need out of their food. They always indicate that vitamins should not be a substitute for food. Certainly this last point is true but how many people eat well enough not need more than that cheese burger or some other fast food?

For years I suffered from migraine headaches but after a couple of years of a well developed vitamin period, the migraines have disappeared. One of the reasons that a short term use of vitamins may not work is that it takes some time to build up and most will just not stay with the time long enough to get it work. As I said for me it took 2 years. For many there is a definite cause and effect relationship in the use of vitamins to help their well being. I will deal with the specifics of this in another article. It must be added here that the FDA does not refer vitamins and supplements, but they are being tested and studied by world-class doctors.

For now, here are three supplements that have value:

1. Vitamin D3. Vitamin D is essential for our bodies. We can not make it in our bodies, so we must get it our diet, in supplements and have the sun make it. Most of us know that “D” helps form and strengthen bones by allowing our body to absorb calcium. Without adequate vitamin D, bones become thin and brittle. The vitamin also helps our muscles and immune system function. The form vitamin D that has shown the most helpful in several studies is D3. A deficiency in vitamin D appears to play a role in several medical conditions, including- Cancer (breast, prostate and colon), Diabetes, Heart Disease, Osteoporosis, and Parathyroid Tumors. So, no claims can be made without more study but vitamin D appears to have real value.

2. Vitamin B12. Vitamin B12 works with the B vitamin folate to make our body's genetic material.It helps keep levels of the amino acid homocysteine ​​in check, which helps decrease heart disease risk, and is essential to the production of red blood cells, which carry oxygen through the blood to the body's tissues. Vitamin B12 deficiency can have a number of possible causes, but typically it occurs in those who do not adequately absorb the vitamin from the foods they eat. This can be caused by Pernicious Anemia (where there is a lack of protein), Atrophic Gastritis (a thinning of the stomach lining- which affects 30% of people over age 50), Surgery-where part of the stomach and / or small intestine is removed, Conditions affecting the small intestine (like Crohn's, Celiac, bacterial growth or a parasite), Excessive Alcohol consumption, Autoimmune Disorders (like Graves and Lupus), and long-term us of acid = reducing drugs. There are many doctors who test for low levels of B12 and my friend is one of the best at doing this testing.

3. CO Q-10. Co Q-10 is the spark that keeps our mitochondria pumping out the energy we need every day. Mitochondria are inside our cells and are tiny “energy factors”. Healthy CO Q-10 levels are essential for good heart health. As we get older our CO Q-10 levels decline and they also declare with the use of statin drugs. CO Q-10 is found in the heart, liver, and pancreas. It is found (in small amounts) in meats and seafood. It is often used as a medicine to treat heart and blood vessel conditions, such as congestive heart failure, chest pains, high blood pressure, and heart problems linked to certain cancer disease, muscular dystrophy, chronic fatigue, and Lyme disease. There are even a few other medical uses. Coenzyme Q-10 has been tried for increasing life span- more studies continue. This is due to the fact that higher levels of CO Q-10 are found in those who live longer. CO Q-10 was first identified in 1957. Today millions of people in Japan use it for heart disease. So it appears that CO Q-10 has real value.

Consumer Safety and Supplement Access

Do supplements work? Does it really do what it says? I heard it on TV. If you have ever searched for a supplement on the internet then it is likely you have been exposed to some form of false advertising. One area that is truly bad is prostate supplies or prostate formulas. Beware views that offer comparison reviews and then selectively claim that this product is # 1. Almost all of these sites receive compensation from the makers of the products recommended. The staff and / or website owners have no credentials or qualifications to review or perform any form of quantitative analysis. If you like the freedom to purchase supplements, without a prescription, then it's time to step up and actually help the organizations in this article to do a better job at protecting all consumers from false advertising, false treatment claims, and misleading representations and information. If we fail then legislators will have no choice but to restrict everyone's access to all supplements as a group.

This article applies to all supplements that consumers' freely access in the market place. To help the reader understand I have chosen the category of men's health supplements since it ranks almost at high as weightloss products with respect to misinformation. Due to this, prostate supplements will be used in my examples. For anyone who has ever purchased a prostate supplement from an advertisement or has performed a search on the internet it is most likely you were exposed to misinformation.

Every day people all across the North American continent are exposed to celebrities claiming this or that supplement will change your life. We witness actors in lab coats who appear to be doctors. We are enticed to call now and as a result promised a free bottle of this miracle prostate product. Often the free product promised is not free because consumers are signed up to be automatically billed and shipped product. In order to do this, businesses store the consumer's credit card information. Before you buy any supplement over the internet or on the television it is best to check if other consumers have complained about the product or the company. Most of us are familiar with the non-profit organization the Better Business Bureau (BBB). It is important to look at the number of complaints that have been filed against a specific business. Often businesses can have an “A” rating yet have hundreds of consumer complaints. An extremely popular website allows users to search on a product name and users to read the complaints that have been filed.

In the background there are multiple organizations that do their best to protect consumers. One of these organizations is the Council for Responsible Nutrition (CRN) and another is the National Advertising Division® (NAD®) of the Council of Better Business Bureaus. Recently the Advertising Self-Regulatory Council, the Council for Responsible Nutrition was notably active in the activities of a famous celebrity television physician and the United States Congress. Another organization, National Advertising Division® or NAD® reviews national advertising for truthfulness and accuracy. Their policies are established by the Advertising Self-Regulatory Council (ASRC). This organization reviews national advertising on broadcast or cable television, in radio, magazines and newspapers, on the Internet (websites) or commercial on-line services, or advertising provided direct to the home or office. This organization focuses on product performance claims, superior claims against competitive products and all kinds of scientific and technical claims. In March of 2009, the manufacturer of the product Best Prostate was contacted to participate in a voluntary review of its official product website. Every sentence on the website was examined for product claims, superior claims, scientific claims and or technical claims. Not everything was perfect and some information was brought into full compliance. This review process was a 6 to 9 month process. It is my belief that all manufacturers' product websites should go through this process in the interest of consumer safety and truth in advertising. If this was the case then consumer freedom to access dietary supplements would not be in jeopardy.

The National Products Foundation, a not-for-profit 501 (c) (3) corporation, is also actively involved in consumer safety and truth in advertising. The organization also promotes research and education related to natural products. In 2013, this organization reviewed 275 advertising cases, mailed 150 warning letters, and referred 40 cases to the FTC and the FDA. Year to date 2014 an additional 190 advertising cases have been initiated. This is in addition to 99 warning letters sent to companies. Surprisingly only 70 percent of the organizations contacted made changes to bring their promotions into compliance. This is likely initiated additional submissions to federal regulatory agencies.

In several cases supplement manufacturers make actual treatment claims for a disease state. This is illegal. When the claim can actually cause harm the Food and Drug Administration (FDA) will actually send a warning letter or enforcement action to the manufacturer. These are readily available to consumers by doing a simple search on the internet. If you are ingesting or swallowing a tablet, capsule, or any fluid, it is important to know who you are buying from, and if they have a history of, or have received enforcement actions from the FDA. There are two very easy ways to check this. The first way is to do a search and enter “product name” and then “FDA” on the same line. Then perform the search. You can also substitution the “product name” and use the “Company Name” and then “FDA” and perform the search. If the search shows an entry from ” ” then you should review it to consider the integrity and honesty of the manufacturer. When it comes to consumer safety manufacturers have a responsibility to be straightforward and honest. It is up to all of us to protect our freedoms. On the FDA website you can report a problem by calling the FDA in your local area please familiarize yourself with the type of complaints they accept.

In 2008 it was estimated that approximately 30 percent of men in North America diagnosed with a prostate disease used some complementary or alternative therapy primarily herbal agents, vitamins, supplements, and / or dietary intervention. It is now six years later and that number has increased significantly. This is partly due to the FDA warning issued in 2011 which found that in two large clinical trials with (5-alpha reductase inhibitors) prescription prostate medications that actually showed an increased incidence of high-grade prostate cancer. In addition every night our televisions tell us of yet another prescription medication with a dangerous side effect and that if you or a loved one experienced this or died you need to please contact the law firm. Who would not be a bit distrustful? Then the next commercial promotions the next latest and greatest prescription medication only to have a laundry list of side effects that is quickly read to us and then we are encouraged to ask our physician to prescribe it. It is no wonder why more and more consumers are embracing complementary or alternative therapies.

North American urologists and physicians in general have been slower than their European colleges to use or even understand these complementary or alternative therapies. In Italy, 50% of the indications used by doctors for benign prostatic hyperplasia (BPH) are (plant-based) phyto therapies. While in Germany and other European countries, phyto therapies are first-line treatment for mild-to-moderate enlarged prostate or benign prostatic hyperplasia / lower urinary tract symptoms (BPH / LUTS).

Despite the positive benefits of using the phytosterols (beta-sitosterol) and / or other complementary or alternative therapies, the use of some nutrients in prostatic disease have had less desirable consequences, showing lack of efficiency, adulteration, and / or severe side effects or drug interactions. The AUA (American Urological Association) has encouraged studies of the supplements being used by consumers. Despite this some doctors simply dismiss complementary and alternative therapies. In addition studies of phytosterols, with respect to an enlarged prostate, have been published in the same medical journals as their prescription counterparts. Examples: Lancet, British Journal of Urology, and Cochrane Database of Systematic Reviews. This can be confirmed by visiting NIH.GOV and simply entering the “ingredient” (beta-sitosterol) followed by “problem or disease state” (example: BPH). Respectfully with prescription medications The FDA requires adequate safety data for marketing, yet the number of estimated deaths for all adverse drug reactions and medication errors make these the 5th leading cause of death here in the United States. 2

Although many in the medical community here in North America level charges against complementary and alternative therapies and allege that they are unsafe. Statistics actually support that traditional prescription medications are guilty of being unsafe despite the submission of safety data to the FDA. With respect to prostate problems urologists do not needarily treat BPH or prostatitis pharmaceutically based only on objective parameters. In fact, it is acknowledged that typically they treat these conditions by observation. For physicians to truly understand the benefits derived from nutraceuticals or alternative therapies in terms of enlarged prostate symptom improvements (or other medical condition) they must be aware and familiar with the published literature.

As we acknowledge the 20th anniversary of Congress passing the DSHEA, the Dietary Supplement Health and Education Act. With this, Congress definitively stated that dietary supplements are a category of food, and not subject to FDA pre-approval thebyy allowing your freedom to access complementary and alternative therapies. What is important is that consumers do not assume that all supplement manufacturers practice truth in advertising. It is important to verify and look at the product's reputation and the manufacturer's reputation through the use of objective sources. Another new scheme is companies that new and just appear on the scene. Do a search on the company name. The owners should be disclosed as should the owner of a website. This is not the industry to hide behind an annonymous proxy company. You should be able to call the company and speak to someone. Always consult and inform your health care provider when you initiate any complementary or alternative therapy.
